Grip with front toe by dipping the foot in/down. Hop directly up and swing shoulders first, lift knees as they will follow through the 180. As you're 180ing spot/look at the end of the rail as you come around, this will help with staying centered. Land bolts and ride away like a g. It honestly comes with practice. Spend a few days doing backswaps/270's on every rail, urban lips, tubes, boxes, disasters etc. It'll help a ton
Jump backwards a bit or pedal with you back foot. You're probably leaning too far on one side of the rail either before the swap or when you do it so consciously try to jump back, a frontside pedal with your back foot will help.
Definitely post a video, but for me I always had issue being comfortable with my shoulders being where they needed to be. I think the answer is most likely just doing reps on a tube and if you keep coming off one side, make a big correction that you know is too far so you come off the same side. Then work on dialing it back and finding out where that balance point is.
