SkiBum.Orange Bubble at Park City/Canyons. Directly over their man made snowmaking pond. You fall off over pond. You die. Horrible design. But so is everything at Canyons.

Ryan5_MNLutsen in northern MN has a 2 seater that has the arm of the chair attached in the middle, with no arm rests on the sides and a low back rest. Basically like an old rope swing with a 2x4 hanging on it.
Not super sketchy since it doesn't get too high and is still in decent shape, but it's pretty long and slow, taking stuff out of your pockets or moving around is a bit more tedious.

SuspiciousFishSnow Ghost at Schweitzer can get pretty sketchy at times. Its a classic middle bar double that goes at least 30' over rocks right before the midway unload and often times there is zero visibility.

also the Zugspitze tram, right before they closed it for wind hold. Has a stupid high drop below. Like a thousand feet.
