My question is, do pros get bootfitted? Or do they just sign with a company and deal with pain, and try to make the boots fit?

A_Burgerfitters can change the fit by grinding out the shell and stuff, with such modifications most companies can probably cover the wide range of feet their athletes have
Matthew-You can get custom liners, foot beds, insoles, canting, and shell mods on your boot. Its pretty much impossible for the boot not to fit a pro because of all the mods you can make to a boot.
