Done, some things though..
-There's no 'partner' option, just married or single, essentially, to choose from. I'm sure many people, especially on this site, and elsewhere, aren't either of those.
-Agnosticism and atheism aren't religions
-The agree/disagree scale is weird. It implies that somewhat agree and somewhat disagree are stronger answers than agree and disagree, respectively. Also agreeing or disagreeing doesn't suit a few of the questions, and could be replaced with something better, imo.

I'm not sure if there's any way to touch these up, but w/e - being a bit picky I suppose, it just seems a little rushed

What is she studying, by the way?
