Looking for the best sandbox game

I'm looking for a sandbox game for PC with hopefully thousands of items or at least hundreds. I would definitely prefer if it would have a realistic physics engine and also the price does not matter.

Any ideas or leads are appreciated ^>^

realistic graphics engine.....not really at all  but ya its sick. Or get gta 4 and just modd the shit out of it
