I got one for my last knee surgery. They gave me a local anesthetic before the actual spinal tap, so it wasn't unbearably painful. The only issue I had with it was not being able to sit upright for a few days afterward without getting nauseous, but it's hard to say if that was the spinal tap or something else in my system from the surgery. Just make sure you don't see the needle beforehand and you'll be fine.
