Dog is dying :(

Last couple weeks have been tough on my dog, she's an 11 year old Brittany Spaniel, she has been healthy until the past few weeks, but she suddenly lost a bunch of weight and is now leaking urine, it's incredibly sad, she's a good friend and I think that her time may be coming :(
