haven't been there yet personally (i do need to get some shit checked out ASAP) but the orthopedic specialty hospital (TOSH) which i believe is in murray is supposed to be the best pretty much anywhere. i assume they do PT and know what they are doing. i had a good experience at the U fixing up my broken leg this summer too.

shameful plug for where i work at. intermountain memorial clinic, address is 2100 s 900 e. I'm an MA in the urgent care but I know a lot of the physical therapists and they are great people who are great at their job. recommend it for sure, right in the heart of sugar house. alway accepting new pts. i am not sure on how long the waiting list is, google the location and you should find the # for the pt department. good luck with it all regardless where ya go.

if you're still looking, i 2nd TOSH. i went there when i tore a quadriceps muscle and they were absolutely incredible. TOSH is probably the best or one of the best not only in utah, but in the west, they've worked with a TON of professional athletes and people like me, so they have experience with everything on the spectrum.
