Entries must include a signed release, completed official entry form, promotional video in DVD or Quicktime format, and a
signed release (along with parent or guardian signature for applicants under age 18). All resumes, trick lists, VHS tapes
and still photos will be completely ignored and immediately forwarded to Warren Miller Entertainment.
