some of the best ones that come to mind:
Shai Hulud covering Linoleum is definatley intense
Mighty Mighty Bosstones covering Enter Sandman is pretty good
Tori Amos covering Smells Like Teen Spirit (just cluster fucks you, you wanna scream like you know the song would want you to, but you can because of how she's singing it)
Me First and the Gimmie Gimmes have a lot of good covers
Godhead (I believe) singing Amazing Grace is nuts

and I almost forgot Mr Bungle covering Oops I did it again, I almost pissed myself the first time I heard it.

Way too many other good covers out there though.
