Two tier health care system

What countries have two tier health systems? One that has basic universal care for everybody but advanced care (elective surgeries, etc) for those willing to pay more (or have their insurance pay). If you're country has something like this, what are the benefits/disadvantages in your opinion?

no, just curious. I know Canada has a two tier system to an extent, but I want to know what it's like for those who live within the system, not just read what wikipedia or whatever has to say on it.

well i live in canada. i don't know what u want to know. i don't use the healthcare shit that often. but i can tell u that surgery and all that shit is free. bawlin

I just want to know IF you guys actually do have a two tier system (I looked it up and it looks like it's an issue in Canada, but I don't think it's actually happening, now that I look harder) and what the benefits/disadvantages of it are. Same as any other program - what's good and what's bad.
I'm curious because I was thinking about how everybody deserves health care but then I thought about waiting lists for elective surgeries and have heard that people in Canada can wait up to 6 months for an ACL reconstruction and am just curious.
