The most fucked up film I've seen is called 'Oldboy'. It's a Korean movie, and it's not gruesome (though it is violent), with loads of rape or anything, just really psychologically messed up.

a ton of people are saying requiem for a dream which in my opinion is one of the greatest movies ive ever seen. i used to think it was extremely fucked up as well but then i saw this movie...
couldnt even sit through the whole thing

i turned it off after the wife graphically cuts off her clit with scissors and then smashes her husbands nuts with a sledge hammer and then continues to jack him off while he is unconscious until blood mixed with semen come out. most fucked thing ever...

Requiem is def a good one.   Its had to sit through that movie when the addiction sets in.    Kinda surprised SPUN hasnt been said yet.  

Its kinda like requiem for a dream except with crystal meth.   Mickey Rourke as the meth cook.   just plain creepy.

I was waiting for someone to say Antichrist. I haven't seen the serbian film, but it sounds like it's some extremly fucked up porn. Antichrist was a bit more psychologically disturbing. As well as the part you described which was messed up.

I watched it at a film festival, never seen so many people get up and leave.

in cannibal holocaust there are scenes of actual political executions. in the movie there said as staged. but they in fact are real... quite fucked..
